What is a D4 medical for motorhome owners and Ambulance drivers?
This is a medical assessment which all motorhome drivers and ambulance drivers must pass before being granted their C1 license.
If you passed your driving test before 1997 you will automatically have a C1 license until the age of 70. It will then need to be renewed every 3 years. You will be required to complete a D4 medical assessment if you passed after 1997 and would like to drive your motor home.
What is a D4 medical for forklift and crane operators?
When considering a role involving cranes and forklifts, all operators must prove they are fit to operate heavy machinery and function at various heights. We carry out the driver medical to a group 2 driver medical standard which is that of a professional HGV or Bus driver using DVLA standards.
Although there is no legal obligation to obtain this medical but a fitness certificate is required by most employers that you are declared medically fit by a registered GP.
What does the D4 medical include?
The medical examination consists of an eyesight test, blood pressure checks to ensure your BP meets council criteria and a questionnaire regarding your medical history. Our doctor will review your medical records in line with council requirements, some councils require full medical records while others simply require a summary. The duration of the visit should take no longer than 30 minutes.

What do I need to bring to the appointment?
Please bring:
- D4 Medical Form ( you can order it from here)
- Photo ID
- Your driving glasses/contact lenses & prescription
- Details of any ongoing medical conditions plus any medication you are taking
- Medical records if you suffer from any preexisting medical condition
